Once we receive your enrollment form, we will submit directly to RX Outreach and you will receive confirmation within 2 business days. When confirmed you are enrolled, you will receive additional instructions from RX Outreach on where to have your doctor send in your prescriptions:

 

Please note, once enrolled, until your doctor MUST send in a prescription otherwise you will not get your medication!!!

Prescriptions may be written with refills available for up to one year.

Ask your doctor about a 180-day supply with one refill or a 90-day supply with three refills.

 

Also, please make sure that your prescriptions include the following items:

1. First and Last Name

2. Shipping address

3. DOB (Date of Birth)

4. SSN (if applicable)

 

If you provided a credit card have your physician ePrescribe (NCPDP 2635855) or fax the prescription to 800-875-6591

(To prevent delays in processing your order, prescriptions can ONLY be faxed from a physician's office)

To order controlled substances, you must fax or mail a copy of your Photo ID Card (for example, a driver's license or state ID card) AND a copy of your Social Security Card or Green Card. Controlled substances and non-controlled medications will ship separately. We cannot ship controlled substances to a P.O. box or a doctor's office.

 

Controlled Substances are: Alprazolam, Androxy, Armodafinil, Belviq, Chloridiazepoxide, Clonazepam, Dexmethylphenidate, Dextroamphetamine, Diazepam, Diphenoxylate/Atropine, Eszopiclone, Flurazepam, Lorazepam, Methylphenidate, Modafinil, Oxandrolone, Phentermine, Testosterone, Temazepam, Tramadol, Zaleplon, and Zolpidem.
